Please navigate to the URL or scan the QR code provided on your welcome card. This will bring you to your patient portal.
To create your patient account, you will receive a one-time link. This link is unique to you and ensures the security of your account.
To begin, enter the email you want to use for your account and select Continue with email. A one-time link to complete your account will be sent to that email address.
TIP: Be sure to double-check the spelling of your email address when signing up. If it's incorrect, you won't receive the link to complete your account or be able to log in later.
Once you receive the one-time link to your email, click on the provided link. This will direct you to a secure page to set up your account. You will be prompted to input some basic information for lab processing purposes. Once you have input all the information, click "CONTINUE", and you will be directed to the homepage. Click the "Register Your Kit" button on the right-hand side to complete the registration process.
